Maternity model call questionnaire

Thank you for your interest in participating in our model call!

Please read the rules of model call below, before submitting the questionnaire!

To be considered for a model call photo session, please fill out this questionnaire.

1. The purpose of this model call is to try out new poses, props and setups to add to photographers portfolio and publish on the website and social media.
2. You will need to sign model release form and agree to use photos from your session in photographers portfolio and on social media.
3. Photographer will choose props, colors and style use for this session.
4. Photographer will take pictures of pregnant person only. No parents, siblings or family posses will be included in the session.
5. Sessions will take place on the following days: Monday Oct. 2nd, 2023 and Wednesday Oct. 4th, 2023 from 9am-12:00pm and each session will take around 15-30 minutes.
6. There is no financial compensation for a model call.
7. Models chosen for the session will receive 5 (five) complimentary digital images.
8. Models will have the opportunity (with no obligation) to purchase additional digital images from their session and will have opportunity (with no obligation) to purchase print products.
9. If you are chosen to participate in the model call, you will be contacted by email.
10. If you are chosen, we require a $20 deposit to secure your spot and this will be the only payment that you will need. No additional payments required, unless you choose to tip your photographer.  If you don't show up without 24 hours notice, deposit will not be returned and you will lose your spot. 
11. Please keep in mind that we will not have any more dates available for this type of session, for that reason we cannot reschedule your session for a different day/time after booking. Please make sure to book for a day/time that you are available.
